Blueprint is suddenly taking a particular statement from another client's session and inserting it into a different client's note, even though the statement doesn't apply and is randomly placed in the note. As therapists, we are expected to proofread the notes generated by Blueprint. Still, it is highly concerning that the system is inserting statements from a different client into another client's session notes. This is not only unethical and clinically inappropriate — it represents a serious breach of confidentiality and could constitute a direct HIPAA violation. Even if we catch the error before signing the note, the fact that this kind of cross-client data mix-up can occur raises major concerns about the security and integrity of the entire system. It compromises the accuracy of clinical documentation and creates a real risk of unauthorized disclosure of Protected Health Information (PHI), which could have legal consequences for both therapists and the organization. Please fix this.
